HOME WITH CONFINDENCE

Ease your mind and know that you will be greeted at your doorstep by an experienced physical therapist who will make sure your feel confident on your first day home.

A Comprehensive Home-Safe session includes:

Seamless transition

Flexible scheduling to allow for same day as discharge or next day – attempt made to schedule for meetup as you arrive at home

90-120 minutes session taking the time you need to feel confident

Accessibility Assessment

Assess you home for ease in using your mobility aids (walker, cane, wheelchair, etc) and give recommendations to enhance independence and safety
Assist with home entry — stairs, thresholds, ramps, uneven surfaces
Assist with car transfer

Mobility Safety Evaluation

Assess home mobility safety
Identify fall risks
Suggest adjustments for safety and independence
Bring simulation to real life!
- Practice all applicable mobility tasks— walking, transferring in and out of your bed or on and off your couch, toilet transfers, shower/tub transfers, wheelchair mobility
- Hands on practice of mobility tasks with family or caregivers to enhance safety
- Recommendations for safe mobility and exercise in your home to reduce fall risk and promote activity

You will have access to your PT for questions or concerns to be answered via text, phone, or email for 1 week.

A follow up email will be sent with reminders of recommendations and tips.

How long is this session?

Typically, this session will range from 90-120 minutes. We will take the time needed to make sure you feel more confident in navigating life at home. You likely simulated all these tasks in inpatient rehab or in the hospital, but it can be overwhelming to do them for the first time without a therapist in your space. We give you all our tips and tricks to help you have an easy and safe transition home.
